[robocup-rescue-s] Travel suport form

Hi Robocuppers,

please note that you can get money from the RoboCup community to cover
your travel expanses to Germany.
To be within the selection of those who will get travel support, it is
absolutely necessary that you fillout the following travel support form
and send it with your qualification metrial as already announced.

We do only accept one form for one team. Please do only send one single
form from your team, even you are participating in more than one
competition, e.g. you participate in the agent competition and the
infrastructure competition.
